§ 38-78. Application.

Application for a crematorium license shall be made on forms to be provided by the clerk and shall, in addition to such other information as is required, set forth the name and business address of the applicant, all parties having an interest in the business, which in the case of a corporation shall include the officers and directors of the corporation, the type of license applied for, and the number, type and proposed location of devices intended to be used. Each application shall include such additional identifying information as required by city staff concerning all parties having an interest in the business.
